 A 35 year old female from Suryapet presented to causality with Abdominal pain  , loss of appetite  and SOB.

Chief complaints

 A 35 year old female from Suryapet presented to causality with Abdominal pain since 4 days, loss of appetite since 3 days  SOB due to Acidity and Generalised weakness.

History of present illness

Patient was apparently asymptomatic 4 days ago. Then she developed chief complaints of red urine output, intermittent  fever with chills accompanied by indigestion and bloating sensation for which she sought medical help and was found to have mild hepatosplenomegaly and was treated conservatively. But her c/o pain in abdomen haven't been resolved.

No c/o loose stools/ vomitings / cold / cough

History of past illness

History of hysterectomy 10 years back.

No HTN / DM / THYROID/CAD/ epilepsy

Normal apetite

Mixed diet

Regular Bowels

Normal Micturition

Teetotaller 

Family History

NO family member  presented with any significant disease.


Vitals 

BP- 110/80 MM OF HG

HR 120

General Examination 

No icterus

No cyanosis

No pedal oedema

No clubbing of fingers

No lymphadenopathy

SYSTEMIC EXAMINATION

 A. CVS

- No thrills
- S1 S2 positive
-no cardiac murmurs

B. RESPIRATORY SYSTEM 

- no dyspnoea 

- No wheezing

- central trachea

- vehicular breath sounds 

C. ABDOMEN

-  scaphoid shaped abdomen 

- No tenderness

- No palpable mass

- non palpable liver and spleen

C. CNS

- conscious and coherent

- normal speech
